Special Education Forms and Instructions: Required Notice and
Consent Forms and Instructions represents the July 2007 update of
this document, bringing Illinois into compliance with IDEA 2004 and
23 Illinois Administrative Code Part 226. All previous version of
these forms and notices are now obsolete. These are the official
versions of the state-required forms. These forms are used by
Special Education Administration.
