PARENT MENTOR IMPACT SURVEY:

The ISBE and the Parent Mentor Project want to learn more about the experiences of parents with children who have an Individualized Education Program (IEP) and the impact of the services of the Parent Mentor working in your area. To take this survey online, you will need to know the name of the school district that operates the Parent Mentor Project in your area. The survey will take 10-15 minutes to complete.
